Northwest Treatment in Oregon City, OR has partnered with a data analytic company, Commonly Well, to conduct a survey looking at the connection between Mental Health & Relationships. This includes our family, friends, romantic partners, our alcohol and drug use, our mental health struggles and all aspects connecting these in our lives.

I am hoping that you will be willing to take 8 minutes of your time to fill out the attached/linked survey. There is also a part 2 of this survey that is texted to you if you provide your cell number on the last page of the survey. This second part of the survey deals with the Recovery Capital Index which is a validated assessment for resiliency and wellness in regards to addiction. Your Recovery Capital Index score is shared immediately with you.

I have attached the flyer, postcards and image and I hope you are willing to share this with your community & connections. We are hoping to share this survey far and wide to anyone that lives or works in Oregon. The survey will be open till the end of July and we will be posting and sharing the results publicly with the hope of improving the education and training for behavioral health therapists in Oregon.
